Practical information
Information to prepare for your outpatient visit
In most cases, a treatment under local anesthesia is scheduled immediately. An outpatient treatment typically lasts about 20 minutes. In some cases, an initial consultation or examination takes place first, after which a separate treatment appointment may be scheduled if necessary. If you are under 18 or require authorized supervision, you may bring one person with you for support in the outpatient treatment room.
What to bring
Please note: this information is very important. If you do not have certain documents with you, it may mean that you cannot be treated.
For your appointment, we ask you to bring the following:
-
your passport or ID card
-
your health insurance card
-
your referral letter (from your doctor, dentist, or medical specialist)
-
a completed list of your relevant medical history
-
an overview of your current medication
Before the treatment, you will need to sign an 'informed consent' form. This is a form in which you acknowledge being aware of the (usually minor) risks of the procedure. These risks will be explained to you both in writing and verbally.
